New Patient Forms:
Please print and fill out new patient information forms from home for your convenience.
For your first visit please consider a few things:
- It is appreciated if you arrive 15 minutes early to help finish any needed paperwork prior to your appointment.
- Your first appointment will last on average 1 hour so plan accordingly.
- Wear or bring clothing that is loose-fitting to allow comfortable movement during your evaluation.
- Bring whatever referral paperwork given to you by your Doctor.
- Your Therapist has set aside time for you, so please call 24 hours in advance of any cancellation.
